Consultation
A formal consultation to answer a clinical question or perform a procedure limited to one or a few visits.
Co-Management
Co-management of diabetes care with current primary care provider or endocrinologist. This is the default relationship unless otherwise specified at the time of referral.
Co-management with principal care for diabetes by the Diabetes Center.
Transfer of Care
Transfer of patient care to the diabetes center for coordination of care as it relates to diabetes and related complications, after discussion and agreement between the diabetes center provider and referring provider.
Individual Diabetes Self-Management Education and Support
1:1 education sessions between you and the diabetes care & education nurse to address your current concerns. Diabetes related discussion topics include:
- Navigating a new diagnosis
- Blood Glucose Monitoring
- Continuous Glucose Monitor (CGM) technology
- Diabetes Medications
- Insulin administration
- Healthy Eating
- Staying Active
- Low and high blood sugar management
- Preventing Complications
- Healthy Coping and Stress Management
- Insulin Pump/Pod start (with provider order and prior consultation)
- Goal Setting
Group Sessions
Type 1 Diabetes MeetUps:
Who?
Adults in the Central Peninsula Community who live with Type 1 Diabetes and are interested in building connections and having meaningful conversation with others living with T1D.
What?
This is an in-person gathering facilitated by Central Peninsula Diabetes Center to build a supportive community for adults experiencing T1D. No need to sign up – simply show up and start building connections with others living with T1D.
Where?
Gatherings take place in the Kenai Room, on the 3rd Floor of Central Peninsula Hospital’s River Tower.
When?
Meeting time is from 3-4pm and take place every other month, less formal meet-ups are encouraged in the off months as desired by group members.
Why?
It can be hard to find and connect with others experiencing Type 1 Diabetes, especially as an adult. At CPDC we desire to foster community and connections for those living with T1D.

Medical Nutrition Therapy is available through Central Peninsula Hospital’s Registered Dietitians for specific help with therapeutic diet education. Ask for a nutrition referral from your primary healthcare provider to visit with a CPH dietitian.
